About the JobThis full-time opportunity requires 1 year of hospice experience, with a focus on case management and coordination of care. You'll work closely with interdisciplinary teams to develop and implement individualized care plans, ensuring the best possible outcomes for our patients.
We offer a competitive salary, with an estimated annual range of $65,000 - $80,000, based on experience and qualifications. Additionally, you'll enjoy:
