Job SummaryThe successful candidate will provide psychosocial support to patients and families by providing skilled social work intervention. This includes providing resources, referrals, education, supportive counseling, discharge planning, and assistance with final arrangements.
This position requires driving city-wide, and care delivery primarily takes place in the patient's home or community. Weekend availability is required.
Responsibilities- Develops and maintains therapeutic relationships.
- Provides comprehensive psychosocial assessments.
- Develops and maintains comprehensive plans of care.
- Provides supportive counseling.
- Facilitates smooth transitions from care environments.
- Mobilizes community resources.
- Provides effective patient/family/caregiver teaching.
- Supports the death/dying process.
- Collaborates in providing patient care.
- Creates timely and accurate documentation.
- Assures patient safety.
- Master's degree in social work from an accredited school and a minimum of 1-3 years of palliative care or hospice experience.
- Demonstrated ability to assess emotional needs, as well as social needs of patients and their families.
- Ability to administer psychological/emotional assessments and define treatment planning from results.
- Excellent communication and interpersonal skills.
- Ability to effectively collaborate with interdisciplinary teams in providing patient care.
- Solution-driven, creative, and resourceful problem-solving skills.
- Ability to function independently and effectively anticipate needs and problems.
- Skilled in organizing and prioritizing work.
- Ability to manage time well and perform assigned duties with attention to detail, accuracy, and follow-through.
- Must possess a professional image, positive attitude, enthusiasm, and self-motivation.
- Comfortable maintaining professional boundaries.
- Proficient computer skills.
- Broad understanding of community resources, including those related to financial benefits.
- Awareness and sensitivity to cultural diversity.
- Knowledge of self-care, personal boundaries; abilities, limits, and inner resources.
- Current CO Driver's license and automobile insurance.
- Previous hospice experience.
- LCSW licensure preferred.
- Bilingual abilities a plus.
The salary for this position is competitive and commensurate with experience. The pay range for this role in Texas typically falls at $35/hour. Actual compensation may vary based on qualifications, experience, and location within the state.
