Job Summary

The Salvation Army Great Lakes Division is seeking a skilled Case Manager to provide case management and community services for clients. This role involves conducting educational lectures, maintaining case records, and serving as a Case Manager to treatment clients.

Key Responsibilities
  • Case Management: Conduct didactics/educational lectures, maintain case records in accordance with Harbor Light standards, and serve as Case Manager to treatment clients.
  • Support Services: Assist clinical staff with various tasks, participate in the review of clinical issues, activities, and program policies and procedures, and attend all clinical staff meetings and team conferences.
  • Professional Development: Participate in educational courses, seminars, workshops, current literature, and in-service meetings to enhance treatment skills.
Requirements
  • Education: Bachelor's degree in social work or an equivalent comparable human services field.
  • Experience: Experience in case management, mental health, and/or substance abuse populations preferred.
  • Skills: Excellent computer skills, experience using electronic medical records systems, and possession of MCBAP credential (CADC, CADC-M, CAADC) or an approved Development Plan.
