Job Description

At Interfaith Community Services, we empower individuals and families in need by providing essential resources and services with compassion and integrity. Our mission is to foster a community of hope, accountability, and dignity.

We are seeking a skilled Case Manager to join our team in providing supportive services and resources to individuals and families experiencing or at risk of homelessness. As a Case Manager, you will work closely with clients to identify their strengths and barriers to stability, and develop personalized plans to achieve self-sufficiency.

Key Responsibilities:

  • Provide direct services to clients, including diversion, Individual Service Plan (ISP), assessments, crisis intervention, advocacy, and case management
  • Utilize Housing First and Diversion strategies to stabilize clients experiencing or at-risk of experiencing homelessness
  • Administer supportive services per agency and contract guidelines
  • Identify resources and advocate for clients' needs
  • Collaborate with volunteers, interns, and other agency personnel to provide comprehensive support

Requirements:

  • Bachelor's degree in social work or related field, preferred
  • 1+ years of experience working in a housing, social services, or nonprofit setting providing goal-oriented case management/motivational interviewing, preferred
  • Experience working with homeless and/or vulnerable populations, preferred
  • Bilingual (English/Spanish), preferred
