Rapid Rehousing Case Manager Job Description

At Interfaith Community Services, we are seeking a highly motivated and compassionate Rapid Rehousing Case Manager to join our team. As a key member of our homeless services team, you will play a critical role in helping individuals and families achieve stable housing and self-sufficiency.

Key Responsibilities:
  • Identify and coordinate housing for homeless individuals and families, ensuring compliance with federal and local housing regulations.
  • Provide case management services, including goal setting, budget counseling, and housing retention support.
  • Link clients with community-based housing and service resources, including financial assistance funds and housing habitability inspections.
  • Develop and maintain effective communication channels with clients, landlords, and referral sources.
  • Collect and report program data, including HMIS reporting and funder requirements.
Requirements:
  • Bachelor's degree in Social Services or related field preferred.
  • 1+ years of relevant work experience required.
  • Experience working with homeless and/or vulnerable populations preferred.
  • Bilingual (English/Spanish) preferred.
