Job Overview

Interfaith Community Services is seeking a dedicated Case Manager to join our team in providing essential resources and services to individuals and families in need.

This role is a critical part of our mission to empower individuals and families to achieve self-sufficiency and stability.

Key Responsibilities
  • Provide direct services to clients, including intake, assessment, crisis intervention, advocacy, and case management.
  • Complete intake assessments, case notes, treatment plans, and progress notes in accordance with contract requirements.
  • Guide and support clients in identifying their strengths, setting achievable goals, and working towards self-sufficiency.
  • Administer supportive services, link clients with resources, and network with other public and private social service agencies as necessary.
  • Provide services in the community, working with clients in various settings, including other agency programs, faith centers, and school administrations.
  • Read and follow department and agency policies and procedures.
  • Facilitate and coordinate monthly resident meetings, social events, and classes to engage clients.
  • Support the Program Manager in gathering data for program reports.
  • Participate in all staff meetings and trainings.
  • Subscribe to Housing First, Trauma-Informed Care, and Harm Reduction practices when working with clients.
Requirements
  • Bachelor's Degree in Social Work or a related field.
  • 1+ years of work experience in a shelter, social services, or nonprofit setting.
  • Experience working with the homeless population is preferred.
  • Bilingual (Spanish/English) is preferred.
