Job Summary

We are seeking a skilled Case Manager to join our team at Interfaith Community Services. As a Case Manager, you will provide direct services to clients, including intakes, assessments, case management, and client advocacy. Your goal will be to move clients from crisis to self-sufficiency and support the mission of Interfaith Community Services.

Key Responsibilities
  • Provide direct services to clients, including diversion, Individual Service Plan (ISP), assessment, crisis intervention, advocacy, social services, case management, and vocational case management.
  • Assist participants in developing a strength-based/solution-focused, individualized goal and action plan that promotes permanent housing and self-sufficiency.
  • Utilize Housing First and Diversion strategies to stabilize clients experiencing or at-risk of experiencing homelessness and work with participants in locating and securing housing.
  • Coordinate and provide services that are comprehensive, timely, effective, efficient, equitable, and client-centered.
  • Identify resources and advocate for clients' needs.
  • Link clients with resources within the agency and utilize other public and private social service agencies and services as necessary.
Requirements
  • HS Diploma or equivalent and 1+ year experience required.
  • Bachelor's Degree in Social Work or related field preferred or equivalent combination of education and experience preferred.
  • Bilingual (English/Spanish) highly desirable.
  • Experience working with formerly incarcerated individuals preferred.
