Position Overview:

Under the direct oversight of a Registered Nurse or healthcare provider, the Licensed Vocational Nurse (LVN) delivers essential patient care in accordance with San Ysidro Health's established policies and procedures within a clinical environment.

Key Responsibilities:
  • Adheres to San Ysidro Health's policies, core values, and relevant regulations.
  • Executes routine patient care tasks as directed by licensed healthcare professionals, adhering to established clinical guidelines.
  • Conducts comprehensive data collection focusing on various aspects of patient health, including physiological, psychosocial, and cultural factors.
  • Prepares and manages patient charts for upcoming appointments.
  • Effectively delegates tasks and coordinates the activities of the healthcare team.
  • Communicates patient test results and necessary follow-up actions as per established protocols.
  • Engages with patients to record vital signs and other essential information as required.
  • Implements educational strategies to promote patient self-management and identifies learning needs of patients and families.
  • Documents relevant patient information and nursing interventions in the Electronic Health Record (EHR) while ensuring confidentiality.
  • Prepares examination rooms and patients for clinical assessments.
  • Applies infection control measures and aseptic techniques as per policy.
  • Conducts point-of-care testing, including but not limited to urine and pregnancy tests, as instructed.
  • Assists physicians with various diagnostic and therapeutic procedures, ensuring compliance with sterile techniques.
  • Manages inventory of medical supplies and medications, ensuring adequate stock levels throughout the day.
  • Administers medications as prescribed, excluding IV medications, in line with established protocols.
  • Participates in clinical emergency response as assigned.
  • Oversees the sterilization process of medical instruments to meet safety and compliance standards.
  • Coordinates immunization processes and manages daily clinic workflows.
  • Utilizes effective communication skills to foster collaboration within the healthcare team.
  • Demonstrates a strong understanding of safety principles and legal considerations in patient care.
  • Ensures readiness for audits by completing quality assurance tasks and addressing any identified deficiencies.
  • Engages in performance improvement initiatives within the department.
  • Assists with nursing schedules and exhibits flexibility in staffing adjustments.
  • Performs other related duties as assigned.

  • Implements customer service standards as defined by San Ysidro Health.
  • Proactively addresses patient needs and resolves issues within the scope of practice.
  • Responds promptly to patient requests and alarms.

Qualifications:

Preferred Experience:
  • At least one year of experience as a Licensed Vocational Nurse.
Required Education:
  • High School Diploma or GED.
  • Completion of an accredited Licensed Vocational Nursing program.
Required Certifications:
  • Valid California LVN license.
  • Current BLS for Healthcare Provider CPR or CPR/AED certification from the American Heart Association.
Communication Skills:
  • Proficient in reading, writing, and understanding English.
  • Ability to comprehend medical terminology and documentation.
  • Effective verbal and written communication skills.
  • Strong interpersonal abilities.
Work Environment and Physical Demands:
  • Requires continuous standing, walking, and physical activity.
  • May involve lifting, pushing, or transferring patients.
  • Exposure to various clinical conditions.
  • Availability to work evenings and weekends as needed.
  • Reliable transportation is necessary for coverage across clinic locations.
