Job Summary

We are seeking a highly skilled and dedicated Nurse Coordinator to join our team at Great Plains Health. As a key member of our healthcare team, you will be responsible for coordinating care and managing patients in our specialty program and/or department.

Key Responsibilities
  • Coordinate Care: Assume responsibility for the coordination of care and ongoing management of patients in the respective specialty program and/or department.
  • Clinical Liaison: Function as the organization's clinical liaison and resource for the respective specialty program and/or department.
  • Quality Improvement: Participate in PI (Process Improvement) and quality improvement programs and projects that serve to improve the quality and outcomes of the services and care provided to patients in the respective specialty program and/or department.
  • Patient Education: Facilitate patient, family, and caregiver education to promote continuity of care after discharge as well as optimal patient outcomes.
  • Statistical Data: Collect and maintain statistical and other data that is required to be collected and maintained by both the organization and/or others for the respective specialty program and/or department.
Requirements
  • Education: Graduate of an accredited school of registered nursing.
  • Credentials: Must possess a current, valid RN license in state of practice, temporary RN license in state of practice, or compact RN license for current state of practice.
  • Certifications: BLS certification is required. Additional certifications required: ACLS, ENPC or PALS, TNCC, NIHSS, completion of CPI.
Physical Demands
  • Physical Abilities: Able to stand, walk, bend, squat, reach, and stretch frequently. Possess physical agility and adequate reaction time to respond quickly and appropriately to unexpected patient care needs.
  • Visual and Auditory Abilities: Needs adequate hearing and visual acuity, including adequate color vision.
  • Manual Dexterity: Requires fine motor skills, adequate eye-hand coordination, and ability to grasp and handle objects.
Working Conditions
  • Work Environment: May be required to work in a fast-paced environment with frequent interruptions. May be required to work in a clinical setting with exposure to blood and bodily fluids.
