We are seeking a highly skilled and experienced Clinical Program Coordinator to join our team at Great Plains Health. The successful candidate will be responsible for coordinating care and managing patients in our specialty program, as well as facilitating interdisciplinary collaboration and providing clinical liaison services.
Key Responsibilities- Assume leadership role to coordinate care and manage patients in the specialty program
- Facilitate interdisciplinary collaboration and provide clinical liaison services
- Assist in achieving and maintaining certifications and accreditations specific to the specialty program
- Round on patients, assess and monitor continuity of care, and expedite delivery of services
- Facilitate patient, family, and caregiver education to promote continuity of care and optimal patient outcomes
- Collect and maintain statistical and other data required for the specialty program
- Participate in PI and quality improvement programs to improve the quality and outcomes of services and care provided
- Collaborate with physicians, specialists, and other providers to deliver care and services to patients
- Participate in community outreach and education efforts to promote and support the specialty program
- Graduate of an accredited school of registered nursing
- Current, valid RN license in state of practice
- BLS certification
- Additional certifications required: ACLS, ENPC or PALS, TNCC, NIHSS, completion of CPI
