About the Role

Baystate Health is seeking a highly skilled and compassionate Pediatric Palliative Care Spiritual Counselor to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing spiritual care and support to patients and their families.

Key Responsibilities
  • Provide spiritual counseling and support to patients and families, promoting spiritual well-being and thriving based on individualized goals and needs.
  • Develop, implement, and update individualized plans of care according to the goals and wishes of patients and families.
  • Document spiritual counseling in the patient's medical record, adhering to Baystate Home Health and Hospice procedures and policies.
  • Collaborate with the interdisciplinary team to ensure patient and family needs are met, continually evaluating patient response to care.
  • Attend and actively participate in interdisciplinary group meetings, staff meetings, family events, and other meetings as requested.
  • Facilitate linkage to community faith-based leaders as requested by patients and families.
  • Maintain quality, competency, and productivity standards, participating in the measurement of these standards.
  • Develop and provide educational in-service programs for staff regarding spiritual aspects of care, as requested by the manager.
  • Provide emotional and spiritual support to staff and volunteers regarding stress associated with providing pediatric palliative care.
  • Participate in the planning and program for memorial services for Baystate pediatric palliative care families.
  • Provide bereavement services and support to families as requested.
Requirements
  • A minimum of three years of experience as a chaplain, spiritual care counselor, pastoral counselor, or other religious/spiritual counselor, preferably in a healthcare setting.
  • Experience working with individuals in an interfaith environment and respectful of all religious and spiritual beliefs.
  • Experience working with children, teens, and their family members.
  • Knowledge of caring for patients and families in the home setting.
  • Knowledge and experience providing bereavement support.
  • Spanish language skills preferred.
