Baystate Health is seeking a dedicated Pediatric Palliative Care Spiritual Counselor to become an essential member of our home health team.
The Spiritual Care Counselor plays a vital role in the Pediatric Palliative Home Care program, collaborating with a compassionate team of skilled caregivers to alleviate the symptoms, pain, and stress associated with serious illnesses.
Palliative Care teams engage with families of children facing life-limiting illnesses to identify the most supportive path forward.
The primary objective is to enhance the quality of life for both the child and their family in the comfort of their own home.
Position Overview:
The Pediatric Palliative Care (PPC) Spiritual Care Counselor is responsible for providing spiritual assessment and support that aligns with the beliefs and values of patients and their families.
This role employs culturally and linguistically appropriate methods that respect the diverse backgrounds of patients and families.The PPC Spiritual Care Counselor acts as a bridge between community spiritual, religious, and related organizations and the families they serve.
Key Responsibilities:
Deliver spiritual counseling and support in accordance with the patient's care plan, promoting spiritual well-being based on individualized goals and needs while respecting all intellectual, cultural, spiritual, and religious traditions.
Develop, implement, and adjust an individualized care plan based on the goals and preferences of patients and families.
Document spiritual counseling in the patient's medical record following Baystate Home Health and Hospice protocols.Collaborate as part of an interdisciplinary team to ensure the needs of patients and families are met, continuously assessing patient responses to care.
Participate actively in interdisciplinary group meetings, staff gatherings, family events, and other relevant meetings as required for the role.
Facilitate connections to community faith leaders as requested by patients and families.Maintain quality, competency, and productivity standards, contributing to the evaluation of these metrics.
Develop and deliver educational programs for staff regarding the spiritual dimensions of care as requested by management.
Provide emotional and spiritual support to staff and volunteers dealing with the stresses of pediatric palliative care through planned and spontaneous activities as needed and directed by management.
Assist in organizing memorial services for families involved in Baystate pediatric palliative care.Offer bereavement services and support to families upon request.
Required Experience:
A minimum of three years in a role as a chaplain, spiritual care counselor, pastoral counselor, or similar position, preferably within a healthcare environment.
Preferred Experience:
Experience in interfaith settings and respect for all religious and spiritual beliefs.
Experience working with children, adolescents, and their families.
Familiarity with providing care in home settings.
Knowledge and experience in bereavement support.
Proficiency in Spanish is preferred.
Skills and Qualifications:
Proficient in computer information systems with the ability to work independently, possessing excellent verbal and written communication skills, demonstrating competence in organizing and presenting educational programs, and being a good listener with strong interpersonal skills.
