The Spiritual Care Counselor is a vital member of the Pediatric Palliative Home Care team, which comprises a compassionate group of skilled professionals committed to alleviating the symptoms, pain, and stress associated with serious illnesses affecting children.
This role involves collaborating with families of children facing life-limiting conditions to identify the most supportive pathways forward, with the ultimate aim of enhancing the quality of life for both the child and their family within the comfort of their home.
Key Responsibilities:
- Deliver spiritual counseling and support aligned with the patient's care plan, fostering spiritual wellness based on individual needs while respecting diverse cultural, spiritual, and religious beliefs.
- Create, implement, and revise personalized care plans in accordance with the preferences and goals expressed by patients and their families.
- Maintain accurate documentation of spiritual counseling in the patient's medical records, adhering to Baystate Home Health and Hospice policies.
- Collaborate effectively with the interdisciplinary team to ensure comprehensive care that meets the needs of patients and families, continually assessing their responses to care.
- Participate actively in interdisciplinary group meetings, staff gatherings, family events, and other relevant meetings as required.
- Facilitate connections to community spiritual leaders as requested by patients and families.
- Uphold quality, competency, and productivity standards, engaging in the evaluation of these benchmarks.
- Develop and deliver educational programs for staff focused on the spiritual dimensions of care as requested.
- Provide emotional and spiritual support to staff and volunteers dealing with the stresses of pediatric palliative care through organized and spontaneous activities as needed.
- Assist in planning and executing memorial services for families receiving pediatric palliative care.
- Offer bereavement support and services to families upon request.
Qualifications:
Required Experience:
A minimum of three years in a role such as chaplain, spiritual care counselor, pastoral counselor, or similar, preferably within a healthcare environment.
Preferred Experience:
Experience in interfaith settings, working with children and families, knowledge of home care practices, and bereavement support.
Skills:
Proficiency in computer systems, strong communication abilities, competence in organizing educational initiatives, and excellent interpersonal skills.
Education:
Bachelor of Arts (Required)
Certifications:
Driver License
