About the Role

We are looking for an experienced Care Coordinator to join our team at Gastro Health. As a full-time employee, you will be responsible for providing comprehensive care coordination, including managing patient care and coordinating treatment plans.

The ideal candidate will have excellent administrative skills, with experience in medical terminology and electronic medical records. Bilingual skills (English/Spanish) are preferred. A Certified Medical Assistant (AAMA) certification and prior experience working in a healthcare setting are also required.

Job Summary:

  • Provide administrative support for physicians and/or care center providers
  • Manage all administrative tasks and duties for physicians/providers
  • Serve as liaison or coordinator for patients' medical care
  • Streamline patient-physician communications to ensure patient satisfaction
  • Provide medical literature and clinical preparation instructions to patients
  • Assist patients with questions and/or concerns regarding procedures
  • Schedule all procedures to be performed by the physician
  • Review physicians schedule for maximum scheduling efficiency
  • Schedule diagnostic tests, procedures, and follow-up appointments
  • Obtain authorizations for procedures and tests
  • Call patients to confirm procedures a week in advance
  • Schedule follow-up appointments, including recalls
  • Check-out patients at the end of their visit and provide next step instructions
  • Request medical records from doctors and hospitals
