Medical Care Coordinator Specialist

Job Summary:
We are seeking a skilled Medical Care Coordinator Specialist to join our Screening & Prevention program team at Baptist Health Care. As a key member of the team, you will provide assessment and care coordination services to patients, ensuring seamless transitions throughout their healthcare journey.
About the Role:
- Process incoming orders and complete chart reviews to determine patient eligibility based on current screening guidelines.
- Serve as a liaison for the department, communicating with referring offices, physicians, and medical facilities regarding patient concerns, eligibility, and follow-up procedures.
- Act as a clinical resource for patients and providers, offering guidance on procedures, prep instructions, eligibility qualifications, and more.
- Review prep instructions with patients and submit orders for procedure prep under physician supervision.
- Interpret results of scheduled colonoscopy procedures and ensure timely communication of results to patients and ordering providers.
- Administer treatments as ordered by providers and respond to patient prescription requests and phone calls in a timely manner.
- Manage patient recall for repeat colonoscopies and provide directives as specified by performing physicians.
- Accurately document observations, interventions, and evaluations related to client care management and services provided using electronic medical records on a state-of-the-art touch pad tablet.
- Label and provide insurance information for referrals to labs, x-rays, or specialists, adhering to insurance guidelines and protocol.
- Refer patients according to insurance guidelines and record/review services and supplies rendered on superbills for accuracy and proper code selection.
- Complete work efficiently and accurately at a pace consistent with customer flow, utilizing downtime productively to benefit the clinic.
- Assist in providing patient teaching and discharge planning.
- Maintain up-to-date knowledge and certification/licensure requirements.
- Focus on delivering high-quality service and care.
Requirements:
- Technical Diploma/Certificate in Practical Nursing Required
- Licensed Practical Nurse (LPN) licensure in Florida or eligible compact state license Upon Hire Required
- Basic Life Support (BLS) certification from the American Heart Association Upon Hire Required
Skills, Knowledge, and Abilities:
- Excellent verbal and written communication skills
- Proficient in patient care charts and patient histories
- Knowledgeable about patient evaluation and triage procedures
- Familiar with medication administration and patient care standards
- Experienced in clinical and/or surgical operations and documentation
- Skilled in supplies, equipment, and services ordering and inventory control
- Able to read and understand the English language
