Full-time RN Case Manager Job Opportunity

We are seeking a skilled and compassionate Registered Nurse to fill our full-time Hospice RN Case Manager position. Located in the Southern section of the Twin Cities Metro area, this role offers independence and flexibility in providing in-home hospice care to patients.

Compensation: This role comes with an annual salary range of $84,905 - $101,233, making it an attractive opportunity for nurses looking to advance their careers.

Job Description: As a Hospice RN Case Manager, you will be responsible for visiting patients in their homes, assessing their needs, and developing personalized care plans. You will also work closely with an interdisciplinary team to ensure seamless care delivery.

Requirements:

  • A graduate of an accredited school of nursing with a current active license to practice as a RN in the state of MN
  • A minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge
  • One year of hospice care experience preferred but not required
  • Able to travel within a designated geographic area with reliable transportation and a valid driver's license
