Duties and Responsibilities:
  • Develop and implement personalized treatment plans, addressing physical and emotional needs.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.
  • Monitor patients' progress, identifying areas for improvement and making necessary adjustments.
  • Maintain accurate records, adhering to HIPAA guidelines and industry standards.

Qualifications:
  • Current RN or LPN licensure; certifications (e.g., CCM) a plus.
  • Minimum 2 years of experience in substance abuse or mental health settings.
  • Excellent communication, organizational, and problem-solving skills.

Compensation Package:An attractive salary range of $70,000 - $90,000 per annum, depending on qualifications and experience, as well as access to ongoing training and development opportunities within ADDICTION AND MENTAL.
