Job Summary

About the Role:

We are seeking a skilled and compassionate Registered Nurse to join our hospice team as a Hospice RN Visit Nurse.

Responsibilities:

  • Patient Assessment: Conduct thorough assessments of patients' needs, developing personalized care plans to ensure their comfort and well-being during end-of-life care.
  • Symptom Management: Effectively manage pain and symptoms, enhancing patients' quality of life and providing relief.
  • Emotional Support: Offer empathetic support to patients and families, fostering open communication and understanding throughout the hospice journey.
  • Education: Provide valuable information to empower patients and families in making informed decisions about their care.
  • Collaborative Care: Work collaboratively with interdisciplinary teams to deliver holistic and coordinated care for patients.
  • Documentation: Maintain accurate records of patient assessments, care plans, and interventions, adhering to hospice regulations.
  • Family Liaison: Serve as a liaison between patients, families, and healthcare teams, promoting unity and understanding throughout the hospice experience.

Qualifications:

  • RN Licensure: Active registered nurse license, demonstrating proficiency and adherence to nursing standards.
  • End-of-Life Expertise: Deep understanding of end-of-life care principles and practices, with a commitment to providing comfort and dignity.
  • Compassionate Approach: Innate empathy and genuine passion for supporting patients and families during the delicate end-of-life process.
  • Communication Skills: Excellent verbal and written communication skills to effectively connect with patients, families, and interdisciplinary teams.

What We Offer:
