LPN - Hospital at Home Care Coordinator

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our Hospital at Home team. As a key member of our operations team, you will provide day-to-day support to our clinical team and play a critical role in ensuring the success of our Hospital at Home program.
Key Responsibilities- Support the Hospital at Home program operations and logistics, serving as the primary point of contact for patients and families.
- Coordinate patient care under the supervision of the HaH Admitting MD/APP, ensuring seamless transitions and exceptional patient experiences.
- Collaborate with the HaH Hub and hospital-based services to ensure timely and efficient patient care.
- Perform administrative tasks, including supply ordering, inventory management, and logistics coordination.
- Participate in process improvement workgroups and contribute to the development of new initiatives and procedures.
- Graduate of an accredited nursing program.
- Current LPN license in the hiring state.
- Excellent communication and organizational skills.
- Ability to work in a fast-paced, matrix environment.
- Proficiency in Microsoft Office and Teams.
- 1+ years of experience in patient access and delivery systems, or prior healthcare clinical operations experience.
