We are seeking a skilled Registered Nurse to join our team at Abode Hospice and Home Health. As a Home Health Registered Nurse, you will play a vital role in providing high-quality patient care and support to our patients and their families.
Responsibilities- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Initiate and manage patient care plans, including administering medications and treatments as prescribed by physicians.
- Counsel patients and families on meeting their nursing and related needs, and provide education on health and wellness.
- Communicate effectively with physicians and other healthcare professionals to ensure seamless care coordination.
- Ensure compliance with all state and federal regulations, and maintain accurate and up-to-date clinical records.
- Graduate of an accredited nursing school with current licensure to practice in the state of operation.
- At least one year of nursing experience, with three or more years preferred.
- Experience in home health or a similar setting is highly desirable.
- Commitment to clinical and documentation excellence.
