About the Role

We are seeking a highly experienced and skilled Director of Care Management to join our team at Actalent. As a key member of our leadership team, you will be responsible for overseeing the clinical operations of our care management department.

Key Responsibilities
  • Oversee a team of 15 employees, including 4 MSW/LCSW/LMFTs and 2 managers with teams of case managers and community health workers.
  • Distribute time between administrative duties (50-60%) and clinical work (40-50%) as a provider.
  • Collaborate with other professionals to evaluate patients' medical or physical condition and assess client needs.
  • Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation.
  • Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.
  • Plan discharge from care facility to home or other care facility.
  • Modify treatment plans to comply with changes in clients' status.
Requirements
  • LCSW/MSW with 5-8 years' experience in a senior level with at least three years of Behavioral Health admin experience.
  • Must be bilingual (English/Spanish).
  • Will be a provider also with a patient load.
  • Leadership/administrative skills required.
