Clinical Social Services Specialist

As a Clinical Social Services Specialist, you will play a crucial role in promoting the well-being of patients and their families. Your work will focus on addressing the psychosocial aspects of healthcare, ensuring that our patients receive comprehensive support throughout their treatment journey.
Key Responsibilities:
- Provide essential social work support to patients and families navigating illness, hospitalization, and significant life changes.
- Collaborate with a multidisciplinary team to assess and coordinate care, identifying potential needs related to health, finances, and community resources.
- Design and implement effective programs and policies that enhance patient care, utilizing your advanced knowledge and skills.
- Conduct thorough evaluations of selected cases to identify psychosocial barriers that may affect patient outcomes and timely discharges.
- Offer counseling and support to patients and families facing social, emotional, or financial challenges that impact their treatment plans.
- Master's degree in Social Work (MSW) required.
- A minimum of one year of relevant experience in the field of social work.
- Strong ability to collaborate with healthcare professionals and families to develop comprehensive care plans.
- Knowledge of community resources and the healthcare delivery system.
- Proficient in using office software and communication tools.
