Job Summary

The Case Management Supervisor is a key member of the human services leadership team at Community Housing Innovations. This role is responsible for managing and monitoring the development and execution of service and care plans for residents, providing a supportive and empowering environment that fosters stabilization, independence, and self-reliance, and skills development.

Key Responsibilities
  • Lead a team of Case Managers and ensure that service and care plans are up-to-date and customized to resident needs.
  • Coordinate resources and develop partnerships with community-based services to support the team in implementing service and care plans.
  • Provide direct supervision and guidance to team members, including performance evaluations and coaching.
  • Ensure residents receive quality and appropriate services per organizational principles and program requirements.
  • Develop and maintain a resource network for referrals as related to service needs of residents.
  • Ensure Case Managers leverage external services in service and care plan development and implementation.
Requirements
  • Bachelor's degree in social work or psychology required.
  • Five (5) years of experience plus two (2) years of supervisory experience in social services for clients.
  • Experience with immigrants and immigration issues prioritized.
  • Professional verbal and written communication skills.
  • Proficiency in English required, multi-lingual and Spanish proficiency prioritized.
