The Rehabilitation Specialist will play a vital role within the IDT, actively engaging in care planning and working alongside the clinical team.


The Rehabilitation Specialist will be dedicated to coordinating and delivering restorative and rehabilitative therapy services to participants both at the Center and in their homes.

In partnership with the Primary Care Physician, rehabilitation team, and other IDT members, the Rehabilitation Specialist aims to enhance participant independence and safety while fostering the development or restoration of functional abilities.


Key Responsibilities:
Conduct comprehensive therapy assessments in our facility and in-home settings to identify the therapy needs, preferences, and objectives of participants.

Actively engage in IDT meetings to formulate participant care strategies.

Implement and document therapy interventions in accordance with physician directives and/or as established in the participants' care plans.

Administer both active and passive exercises, muscle re-education, gait training, and functional activities of daily living (ADLs); utilize treatments such as massage, heat, sound, water, light, or electrical modalities.

Oversee the Therapy Assistants, providing the necessary training, instruction, and guidance to implement Functional Maintenance Programs (FMP).

Deliver training and instructions to staff, participants, and caregivers/families regarding the use and maintenance of therapeutic equipment.

Qualifications:
Bachelor's Degree in Physical Therapy with a minimum of five (5) years of relevant experience.

A Master's Degree in Physical Therapy or a Doctorate in Physical Therapy is preferred.

Possession of a valid California Physical Therapy License or proof of eligibility for licensure.

At least two (2) years of experience in a clinical setting with a focus on frail or elderly populations is preferred.

Physical therapy degrees obtained outside of the United States must be evaluated for equivalency to a U.S. degree in physical therapy.

Current CPR certification is preferred.

The salary range for this position is $99,309.58 to $119,717.49 annually, plus bonuses and equity. WelbeHealth offers a competitive total rewards package that encompasses 401k matching, healthcare coverage, and a wide array of other benefits.

Actual compensation will be adjusted based on experience and qualifications.
