Home Care Nurse Specialist

As a Home Care Nurse Specialist with Professional Case Management, you will have the opportunity to make a meaningful difference in the lives of our clients. Our company is dedicated to providing exceptional care and support to individuals who require in-home nursing services.
We are currently seeking an experienced Registered Nurse to join our team as a Home Care Nurse Specialist. In this role, you will be responsible for conducting in-home nursing visits for our clients, providing case management, and directing a team of RNs, LPNs, and caregivers in the delivery of care.
The ideal candidate will have a minimum of two years of nursing experience, including one year in home care or a closely related field. A Bachelor's degree in Nursing is preferred, and current, unrestricted RN licensure in the state(s) of practice is required. Additionally, the successful candidate will have strong communication and leadership skills, as well as the ability to work independently and as part of a team.
Key Responsibilities:
- Conduct in-home nursing visits for clients
- Provide case management and coordinate care plans
- Direct a team of RNs, LPNs, and caregivers in the delivery of care
- Collaborate with regional directors and other personnel to identify and correct issues and/or improve services
