Home Health Care Nurse

Aveanna Healthcare is seeking a skilled Home Health Care Nurse to provide compassionate care to our patients in a home setting. As a key member of our team, you will be responsible for implementing physician-prescribed plans of care, conducting patient assessments, and coordinating with healthcare professionals to ensure seamless transitions between settings.
Responsibilities
• Implement physician-prescribed plans of care
• Conduct patient assessments and coordinate care
• Administer prescribed medications, treatments, and therapies
• Maintain timely and compliant documentation of care provided
• Promote health education and training for family members
• Collaborate with Case Manager on client problems, concerns, or changes in client condition
Requirements
• Active, unencumbered RN license in the state where practicing
• Current CPR certification from American Red Cross or American Heart Association
• TB skin test within the last 12 months
• Six months prior hands-on nursing experience preferred
• Reliable transportation required
What We Offer
Aveanna offers a competitive salary range of $65,000 - $80,000 annually, depending on experience, plus benefits such as health, dental, vision, and company-paid life insurance. Our comprehensive package includes paid time off, weekly and/or daily pay, overtime opportunities, electronic documentation, tuition reimbursement, continuing education credits, and a 401(k) savings plan with employer matching.
