Clinical Quality Coordinator

Job Summary:

We are seeking a highly motivated and detail-oriented Clinical Quality Coordinator to join our team at JPS Health Network. As a Clinical Quality Coordinator, you will play a critical role in partnering with the Patient Care Team to improve the quality of patient care and achieve quality-driven goals.

Key Responsibilities:
  1. Partner with physicians and patient care team to assist patients in completing health screens and preventive care measures.
  2. Contact external physician offices to obtain copies of consult notes and reports for patients who had preventive healthcare screens and clinical measures completed at outside facilities.
  3. Remind patients to pick up their prescriptions and identify any barriers with taking prescription or picking up prescriptions.
  4. Update preventive health screens and clinical measures in the Electronic Medical Record (EMR) that have been determined as completed.
  5. Communicate regularly with interdisciplinary team members to coordinate/update a patient's care.
  6. Maintain an accurate tracking log of patients who have been contacted and the status of their preventive health screens and clinical measures.
  7. Participate in various quality-driven initiatives within JPS and collaborate with the team to achieve high-level patient outcomes.
  8. Submit data to external agencies as directed by the Sr. Clinical Quality Coordinator.
  9. Explain the importance of preventive health screens and clinical measures to staff, patients, and families.
  10. Facilitate the identification of patients as high-risk or living with a disease and work with the case management team to address Social Determinants of Health (SDoH).
Requirements:
  • High School Diploma, GED or equivalent.
  • 1 year of experience in the healthcare field.
  • Current certification from a national registry as a Certified Medical Assistant (CMA).
  • OR current licensure to practice vocational nursing in the State of Texas by the Board of Nurse Examiners and must maintain licensure throughout employment.
  • OR requisite educational or training background and 1 year of combined experience in medical billing and coding, patient registration, and navigating patient's electronic medical records may suffice for licensure/certification.
Preferred Qualifications:
  • Associate's Degree from an accredited school of professional nursing.
  • OR successful completion of a Medical Assistant training, coding and billing training program or graduate from an accredited School of Vocational Nursing.
  • Current licensure to practice vocational nursing in the State of Texas by the Board of Nurse Examiners and must maintain licensure throughout employment.
