Job Description

The Clinical Quality Coordinator plays a vital role in partnering with the Patient Care Team to drive quality initiatives and achieve quality-driven goals. This position is based at one of JPS's clinics and works under the direction of the CMS Innovations and Value-Based Care department.

The Clinical Quality Coordinator is responsible for outreach to patients whose clinical measures need to be addressed, in coordination with the JPS Patient Care Team. This involves partnering with JPS outpatient physician offices to help achieve benchmarks for patient care and maintain/improve quality of care delivery across multiple payers and patient populations.

The ideal candidate will have 1 year of experience in the healthcare field and possess a strong understanding of quality improvement principles. A current certification from a national registry as a Certified Medical Assistant (CMA) is required, as well as licensure to practice vocational nursing in the State of Texas. Preferred qualifications include licensure to practice vocational nursing in the State of Texas and successful completion of a Medical Assistant training, coding and billing training program or graduate from an accredited School of Vocational Nursing.

Key Responsibilities
  • Partners with the Patient Care Team to drive quality initiatives and achieve quality-driven goals
  • Outreach to patients whose clinical measures need to be addressed, in coordination with the JPS Patient Care Team
  • Partners with JPS outpatient physician offices to help achieve benchmarks for patient care and maintain/improve quality of care delivery across multiple payers and patient populations
  • Updates preventive health screens and clinical measures in the Electronic Medical Record (EMR) that have been determined as completed
  • Maintains an accurate tracking log of patients who have been contacted and the status of their preventive health screens and clinical measures
  • Participates in various quality-driven initiatives within JPS and collaborates with the Patient Care Team to achieve high-level patient outcomes
  • Submits data to external agencies as directed by Sr. Clinical Quality Coordinator
  • Explains the importance of preventive health screens and clinical measures to staff, patients, and families
  • Facilitates the identification of patients as high-risk or living with a disease and works with the case management team to address Social Determinants of Health (SDoH)
  • Performs quality control checks on equipment, prepares and sterilizes medical equipment, and identifies when others' documentation is incorrect and alerts clinic leadership
  • Coordinates patient care as directed by provider, and policies/procedures
  • Performs telephone and in-person screening limited to intake and gathering of information without requiring the exercise of judgment based on clinical knowledge
  • Performs patient outreach and engagement to ensure patient awareness of clinic appointments and adherence to their person-centered plan
