About the Position

We are seeking a skilled Service Coordinator to join our team at Prestera Center for Mental Health.

Job Description:

The Service Coordinator will work with consumers daily to provide oversight in community, home, residential, or other treatment settings. This includes assisting consumers in self-care, self-direction, independence, mobility, and learning as directed by their service/treatment plan.

Key Responsibilities:

  • Assist consumers with daily tasks such as meal preparation, household cleaning, and grocery shopping.
  • Organize and participate in leisure activities with consumers, providing instruction and guidance on appropriate behavior and interactions for the setting involved.
  • Accompany consumers in the community for activities including medical appointments, grocery shopping, attendance at treatment plan meetings, or other planned outings.
  • Monitor consumers in self-administration of medications or administer medications to consumers if indicated to be AMAP certified.
  • Provide education or supportive services to consumers.
  • Obtain and document vital signs as assigned.
  • Document consumer observations and behaviors according to policy.

Requirements:

To be successful in this role, you should have excellent interpersonal skills and the ability to develop trusting relationships with consumers. You should also possess good written and verbal communication skills in conjunction with good time management and organizational skills.

Compensation:

The estimated annual salary for this position is $45,000 - $60,000 based on location and experience. Benefits include comprehensive health insurance, paid time off, and opportunities for professional growth and development.
