Job Summary:

We are seeking a highly motivated Community Health Worker to join our team at Northern Light Health. As a Community Health Worker, you will provide social support, health coaching, and navigation to access healthcare and community-based services. You will work closely with patients to understand their needs and develop personalized plans to improve their health and well-being.

Key Responsibilities:
  • Provide social support and health coaching to patients in multiple settings, including patient homes and communities, healthcare facilities, and community settings.
  • Assist patients with accessing healthcare and community-based services, including referrals to programs and supports.
  • Conduct needs assessments and help patients set health goals.
  • Motivate patients to meet their health goals and provide ongoing support and encouragement.
  • Help patients with social issues, such as homelessness, substance abuse, and hunger.
  • Assist patients with organizing their records, making follow-up appointments, and obtaining prescriptions.
  • Help patients fill out applications for benefits, such as insurance and food stamps.
  • Facilitate support groups for patients and coordinate workshops and education in various areas.
Requirements:
  • Excellent oral and written communication skills.
  • Ability to carry out written and oral instructions.
  • Ability to exercise judgment in the application of professional services.
  • Comfortable with home visits and outreach.
  • Comfortable with computer data entry.
  • Long-time resident of the community/region with good knowledge of local resources.
Working Conditions:
  • Need to drive to perform responsible duties.
  • Work with computers, typing, reading, or writing.
  • Prolonged periods of sitting.
  • Lifting, moving, and loading less than 20 pounds.
