Job Summary:

As a Community Health Worker at Northern Light Health, you will play a vital role in providing social support, health coaching, and navigation to access healthcare and community-based services. You will advocate for systems change, connect patients to high-quality health care services, and help them improve their health and well-being.

Key Responsibilities:

Provide patient-centered care, taking a holistic approach to support patients across multiple domains.
Develop and maintain relationships with patients, families, and community members to promote health and well-being.
Collaborate with healthcare teams to ensure seamless care coordination and communication.
Facilitate referrals to programs and services that support patients' health and social needs.
Conduct needs assessments, set health goals, and motivate patients to achieve them.
Provide education and support to patients on healthy behaviors, disease management, and self-care.

Requirements:

High school diploma or equivalent required; degree in a related field preferred.
Excellent communication, interpersonal, and problem-solving skills.
Ability to work in a fast-paced environment, prioritize tasks, and manage multiple responsibilities.
Strong understanding of community resources, healthcare systems, and social determinants of health.
Ability to maintain confidentiality, handle sensitive information, and work with diverse populations.
