Dietitian - Migrant and Community Health Center

InterCare Health Network is seeking a skilled Dietitian to join our WIC team in Plainwell. As a key member of our team, you will provide direct nutrition services to clients, assess and interpret medical and nutrition histories, and develop individualized care plans.
Key Responsibilities- Provide direct nutrition services to clients, individually and in group classes
- Assess and interpret medical and nutrition histories, discuss current topics of concern and interest to the client, and provide client-centered counseling and education
- Review information with clients, assessing for understanding and readiness for change, and work with clients to set goals
- Assign, explain, and issue appropriate food packages based on individual needs and preferences
- Provide follow-up and continuity of care for individual care plans
- Stay abreast of current information, knowledge, and practices in the field, and participate in all required nutrition and breastfeeding trainings
- Create nutrition education materials and resources upon request
- Refer clients to nutrition, breastfeeding, health, public assistance, and community services as needed in a client-centered environment
- Completion of and/or eligibility to register for the dietetic registration exam
- Ability to use Microsoft Word sufficient to produce simple correspondence and basic reports and documents
- Ability to use MI-WIC system and other computer programs to gather and analyze data, collate and interpret information, and provide information to clients, programs, and colleagues
- Excellent cross-cultural communication skills and the ability to communicate professionally to clients, vendors, business associates, and staff members at all levels
