Role Summary: As a Service Coordinator, you will be an essential part of an interdisciplinary team, delivering adaptable, empathetic, and community-oriented services aimed at fostering stability, recovery, and reintegration for individuals experiencing mental health difficulties, histories of homelessness, and potential substance use challenges. Service Coordinators play a crucial role within Pathways' Assertive Community Treatment (ACT) team, collaborating with a diverse staff to manage a shared caseload of clients while liaising with community partners.
Available Positions: 1
Work Schedule: Full Time, 35-40 hours per week
Application Requirements:
Applicants must submit a Cover Letter and Resume.
Key Responsibilities:
- Build and maintain trusting relationships with clients, providing services that prioritize client choice, centeredness, harm reduction, and strengths-based approaches.
- Offer individualized support and counseling on essential life skills, housing issues, and health maintenance, including medication management, housekeeping, and financial planning.
- Assist clients in navigating the housing process, including unit viewings, move-in support, life skills training, and resolving landlord-client disputes.
- Coordinate and oversee referrals to community services, advocating for client engagement, and managing communications with external agencies as needed.
- Develop a comprehensive understanding of community resources and demonstrate a proactive approach to exploring new or unfamiliar options.
- Attendance at annual All Staff and Programmatic Retreats is expected, unless otherwise excused by management. These retreats occur during regular working hours and are fully compensated.
- Perform additional duties as assigned.
Qualifications:
- Ability to operate within a Housing First, client-centered, harm reduction framework.
- Demonstrated patience, creativity, flexibility, compassion, and sensitivity towards individuals with disabilities and marginalized populations.
- Strong understanding of the challenges faced by individuals with mental health or substance use issues, particularly regarding basic needs such as food, clothing, shelter, and advocacy. Familiarity with the criminal justice system and its impact on individuals with mental illness is advantageous.
- Excellent independent problem-solving skills and a proven ability to approach complex situations intuitively.
- A valid driver's license, reliable personal vehicle for work-related purposes, and current automobile insurance are required.
- Willingness to engage in self-reflection, learning, and practicing new relational approaches.
- Ability to tolerate risk and discomfort.
Compensation:
- Starting salary of $20-$21, with potential for an increase of up to 8.5% after one year of employment.
- A $3000 Hiring Bonus is available.
