Nurse Practitioner, Heart Failure Ambulatory, Division of Cardiovascular Health

Job Summary:
The University of Cincinnati College of Medicine is seeking a skilled Nurse Practitioner to join our team in the Division of Cardiovascular Health & Disease. As a key member of our ambulatory care team, you will provide high-quality patient care and work collaboratively with our interdisciplinary healthcare team to deliver exceptional patient outcomes.
Responsibilities:
- Practice clinical/Patient Care according to professional practice standards and institutional policy/procedure.
- Intake assessment/triage/admission, including pertinent history, physical, psychosocial, and cultural dimensions, is performed and documented according to policy.
- Ongoing assessment, when appropriate, is performed and documented according to policy.
- Assessment demonstrates knowledge of the principles of growth and development, and identification of the patient's needs as they relate to a development level and cultural dimensions.
- Interpretation of the patient's condition and abnormal findings is communicated in a timely manner to the physician and all appropriate members of the interdisciplinary healthcare team.
- Participates in hospital and provider performance improvement activities.
Requirements:
- Master's Degree in Nursing from an accredited nursing program.
- Active Ohio APRN license with prescriptive authority.
- Active Board Certification in appropriate nursing specialty.
- BLS Certification.
- Three (3) years nursing experience.
