We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Post Acute Care team at Helen Porter Rehabilitation & Nursing. As a valued member of our team, you will provide high-quality nursing care to our residents, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.
Responsibilities:
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans, working closely with residents, families, and healthcare team members.
- Participate in interdisciplinary team meetings to discuss resident progress, concerns, and goals.
- Collaborate with other healthcare professionals to ensure seamless transitions of care.
Requirements:
- Graduate of an accredited LPN program.
- Current LPN license or compact license recognized by the State of Vermont.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
Preferred Qualifications:
- Previous long-term care experience, with a focus on post-acute care.
- Basic computer skills, with experience using electronic medical records.
