We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Helen Porter Rehabilitation & Nursing. As an LPN, you will play a vital role in providing high-quality nursing care to our residents, promoting their independence and dignity.
Job SummaryThe LPN will be responsible for providing direct patient care, administering medications, and performing various nursing procedures. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.
Responsibilities- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Conduct thorough assessments of residents' physical and emotional needs.
- Collaborate with other healthcare professionals to ensure seamless care transitions.
- Participate in quality improvement initiatives to enhance resident outcomes.
- Graduate of an accredited LPN program.
- Current LPN license or compact license recognized by the State of Vermont.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Basic computer skills and proficiency in electronic medical records.
- Previous long-term care experience.
- Experience working with geriatric populations.
- Basic life support certification.
