LPN - Skilled Nursing Professional

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Porter Medical Center. As an LPN, you will provide high-quality nursing care to our residents in a long-term care setting. Your primary responsibility will be to assess, plan, and implement individualized care plans to meet the unique needs of each resident.
Responsibilities:
• Provide direct patient care, including administering medications, performing wound care, and assisting with daily living activities.
• Conduct thorough assessments of residents to identify their physical, emotional, and social needs.
• Develop and implement individualized care plans to meet the unique needs of each resident.
• Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
Requirements:
• Graduate of an accredited school of nursing with a current LPN license or compact license recognized by the State of Vermont.
• Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
• Ability to work in a fast-paced environment, with a high level of flexibility and adaptability.
