As an MDS Coordinator, you will be responsible for ensuring the accuracy and timeliness of Minimum Data Set (MDS) assessments, as well as providing education and training to staff on MDS and person-centered care planning.

Responsibilities
  • RAI/MDS Expertise: Possess in-depth knowledge of the RAI/MDS process, including person-centered care planning, Medicare PDPM reimbursement, scheduling, month-end triple check, appeals process, and Medicare certification.
  • Assessment Coordination: Maintain accurate PDPM/Medicare and Medicaid/Private assessment schedules, ensuring proper reference dates for distribution to therapies, dietary, social services, activities, and nursing.
  • Education and Training: Provide ongoing education and training to staff on MDS and person-centered care planning.
  • Regulatory Compliance: Ensure compliance with RAI processes, including MDS assessments, Triggers and CAAS, and Care Planning, in accordance with state, federal, and OBRA regulations.
  • Medicare Certification: Manage the Medicare certification/recertification binder to meet Medicare physician certification requirements for Part A & B services.
