Job Summary

We are seeking a highly motivated and compassionate Activity Assistant to join our team at Grand Rapids SNF, LLC. As an Activity Assistant, you will play a vital role in enhancing the lives of our residents by planning and implementing engaging activities and events that promote socialization, recreation, and overall well-being.

Key Responsibilities
  • Develop and implement activity programs that cater to the diverse needs and interests of our residents, including large groups, small groups, and individual attention.
  • Observe and report resident attendance, participation, and behavior changes, documenting and charting as necessary.
  • Transport residents to and from programs and assist with community trips as scheduled.
  • Prepare for programs by gathering equipment and supplies as needed prior to starting.
  • Collaborate with the Director of Activities to ensure planned activities meet specific resident needs.
  • Encourage resident participation in group programs through one-on-one invitations, announcements, and encouragement during the program.
  • Knowledgeable of individual care plans and support the care planning process by providing supervisors with specific information and observations of resident needs and preferences.
  • Coordinate activity programs with volunteers from the surrounding community, providing guidance and assigning tasks.
  • Meet and greet new residents upon arrival to the facility.
  • Notify nursing personnel immediately if an accident or incident occurs with a resident during a group program or one-on-one room visit.
  • Report to the Charge Nurse or Unit Manager any change noticed in a resident's behavior, mental or physical condition, and report to the Recreation Director any incidents, occurrences, and changes in condition.
  • Adhere to Standard Precautions and the Facility's Infection Control Procedures in all aspects of work performance, and store and use all activity equipment in accordance with good safety practices.
  • Maintain the comfort, privacy, and dignity of residents and interact with them in a manner that displays warmth, respect, and promotes a caring environment.
  • Promote and protect resident rights by assisting residents to make informed decisions, treating residents with dignity and respect, reporting suspected abuse or neglect, avoiding the need for physical restraints in accordance with current professional standards, and supporting independent expression, choice, and decision-making consistent with applicable law and regulation.
  • Communicate and interact effectively and tactfully with residents, visitors, families, peers, and supervisors.
  • Report all hazardous conditions, damaged equipment, and supply issues to the supervisor.
  • Attend and participate in departmental meetings and in-services as directed and complete annual in-services as required by the facility.
  • Maintain a high level of confidentiality in accordance with HIPAA guidelines at all times and protect confidential information by only providing information on a 'need-to-know' basis.
  • Perform other duties or functions as assigned.
Requirements
  • High School Graduate or equivalent required.
  • Experience working with senior groups or in leading/assisting with group activities preferred.
  • Knowledge of and experience with various types of arts and crafts preferred.
Working Conditions

This position requires the ability to work cooperatively with others in the department and facility, and to maintain a high level of confidentiality in accordance with HIPAA guidelines at all times.

The ideal candidate will possess strong interpersonal and communication skills, and be able to work flexible hours, including evenings and weekends.

Bachelor's degree in a related field or equivalent experience is preferred.
