About the Role

We are seeking a dedicated and compassionate Habilitation Assistant to join our team at ADAPT Community Network. As a key member of our support staff, you will play a vital role in providing high-quality care and services to individuals with developmental disabilities.

Key Responsibilities
  • Assist in the implementation of program plans, including goals, ongoing services, and special needs, under the supervision of a Habilitation Specialist.
  • Provide physical and programmatic support to individuals in assigned areas, including daily operations, supervision, and community outings.
  • Drive Agency vehicles for transportation, pickups, deliveries, and medical appointments as required.
  • Perform clerical tasks, such as collating, folding, typing, and answering telephones, as needed.
  • Participate in case conferences and serve as a member of interdisciplinary plans.
  • Collect data for written program plans, including behavior plans and treatment plans.
  • Provide necessary program support and coverage, including writing case notes and reporting incidents.
Requirements
  • Valid New York State Driver's License and good standing.
  • Ability to read, write, speak, and understand English.
  • Good oral and written communication skills.
  • Good interpersonal skills for effective interaction with coworkers, employees, and individuals and their families.
  • Ability to analyze problems and determine corrective measures.
  • Must be vaccinated against Covid-19.
Education and Experience
  • High School Diploma or GED required.
  • Six months of related experience preferred but not required.
  • Salaried employees must be approved Medication Administration Personnel (MAP) at time of hire or within six months of date of hire in salaried status.
