Key Responsibilities- Advocacy and Support: Advocate for and with our members to ensure informed decision making, informed consent, and appropriate guardianship.
- Life Plan Development: Schedule, lead, and collaborate with members and their interdisciplinary team to conduct meetings and assessments, ensuring the development of a comprehensive, person-centered Life Plan that reflects the person's needs and desired life goals.
- Implementation and Monitoring: Implement, update, and monitor Life Plans, and facilitate individualized Life Plan reviews and approval processes.
- Integration of Supports: Ensure integration of all needed and preferred supports and services, including medical, behavioral, social, habilitation, dental, psychosocial, and community-based, and facility-based long-term supports and services.
- Ongoing Contact: Maintain ongoing contact with critical people in a member's life.
- Documentation: Ensure timely submission of all documentation, including Life Plans, Progress notes, etc.
- Benefits Assistance: Assist members with maintaining benefits, such as Social Security, Supplemental Security Income, Medicaid, and Medicare coverage, and Food Stamps.
- Problem Resolution: Assist members to resolve problems in living, such as housing, utilities, the judicial system, and general safety.
- Reporting: Report abuse or neglect immediately when observed or reported.
- A Bachelor's degree with 2+ years of relevant experience, OR
- A License as a Registered Nurse with 2+ years of relevant experience, which can include any employment experience and is not limited to case management/service coordination duties, OR
- A Master's degree with 1+ year of relevant experience.
- Strong communication skills, including verbal and written communication skills, along with strong interpersonal and organizational skills.
- Excellent organizational, interpersonal, and verbal and written communication skills.
- Bilingual in English and Bengali.
