Care Coordination Social Worker (LMSW) Role Summary

The Care Coordination Social Worker (LMSW) is a key member of the Norwalk Hospital Association team, responsible for providing counseling, crisis intervention, and complex discharge planning services to assigned inpatients and patients in designated outpatient areas.

This role requires a strong understanding of community resources and the ability to collaborate with the Clinical Care Coordinator to design and implement effective discharge plans.

Key Responsibilities:

  • Conduct thorough assessments to identify patient needs and develop personalized discharge plans.
  • Collaborate with the Clinical Care Coordinator to implement discharge plans and ensure timely and safe patient transitions.
  • Provide counseling and support to patients and families in crisis, including substance abuse counseling and referrals.
  • Participate in Maternal/Child Health initiatives, including grief counseling and referrals for identified needs.
  • Assess patients for abuse/neglect, domestic violence, and report to appropriate agencies as needed.
  • Lead patient and family conferences as needed, providing Advanced Directive resources and Patient Right to Choose information.
  • Complete readmission and CARL tools as required.
  • Fulfill all compliance responsibilities related to the position.

Requirements:

Requires a Master's Degree in Social Work and a minimum of three years of counseling and discharge planning experience in an acute care hospital or equivalent.

Strong prioritization skills, ability to act independently, and effective communication with physicians, patients, families, and interdisciplinary team members are essential.

Case Management Certification is preferred.
