About the Role:

As a Hospice Nursing Specialist at Trinity Hospice, you will play a vital role in delivering high-quality care to patients in need.

Compensation:

The salary for this role is $60,000 - $80,000 per year.

Job Description:

In this critical role, you will provide expert nursing care to patients of all ages in their place of residence.

You will work collaboratively with the interdisciplinary hospice group, patient, family, and referring agency to develop and implement a plan of care that ensures quality and continuity of care.

Key Responsibilities:
  • Administer high-quality nursing care to patients in their place of residence.
  • Develop and implement individualized plans of care to ensure quality and continuity of care.
  • Determine scope and frequency of services needed based on patient acuity and needs.
Requirements:Education:

A graduate degree from an accredited Diploma, Associate or Baccalaureate School of Nursing is required.

Licensure:

A current registered nurse (RN) license is necessary, and/or must be in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).

Experience:

Prior experience as a Registered Nurse (RN) in a clinical care setting is preferred.

Additional Requirements:

Reliable transportation, a valid Driver's License, auto liability insurance, and current CPR certification are also required.
