Job Title: Home Health Physical Therapist

We are seeking a skilled and compassionate Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a Home Health Physical Therapist, you will play a vital role in providing high-quality care to patients in their homes.

Job Summary:

The successful candidate will be responsible for providing skilled physical therapy services to patients in their homes, as ordered by the attending physician. This includes developing and implementing individualized plans of care, educating patients and their families, and ensuring the safety of patients.

Key Responsibilities:
  • Plan and implement individualized plans of care for patients in their homes
  • Provide skilled physical therapy services, including exercises, stretches, and other interventions as ordered by the attending physician
  • Educate patients and their families on proper use of equipment and techniques for managing their condition
  • Ensure the safety of patients and their families by identifying and mitigating potential risks
  • Collaborate with other healthcare professionals to ensure comprehensive care
Requirements:
  • Current license to practice as a Physical Therapist in the state of employment
  • Valid driver's license and auto insurance
  • Ability to lift and move patients as needed
