Home Health Physical Therapist

We are seeking a highly skilled and compassionate Physical Therapist to join our team at JonSan Home Health Inc. as a Home Health Physical Therapist. As a Physical Therapist, you will play a vital role in providing high-quality physical therapy services to patients in their homes, promoting independence, and improving their overall quality of life.
Responsibilities:- Assess and evaluate patients' physical therapy needs, developing individualized treatment plans to achieve their goals.
- Conduct home visits to evaluate patients' environments and provide recommendations for modifications to ensure safe and effective treatment.
- Direct and implement physical therapy interventions, instructing patients and caregivers on proper techniques and exercises.
- Monitor patients' progress, adjusting treatment plans as needed, and communicating with healthcare teams to ensure seamless care.
- Collaborate with other healthcare professionals to develop and implement comprehensive care plans.
- Participate in case conferences and contribute to the development of patient care plans.
- Provide education and training to patients, caregivers, and other healthcare professionals on physical therapy techniques and best practices.
- Supervise and evaluate Physical Therapy Assistants, ensuring they provide high-quality care and services.
- Master's degree in Physical Therapy from an accredited program.
- Current Physical Therapy License in the state of Texas.
- At least two years of experience as a Physical Therapist, preferably in home health care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Current CPR certification and a valid driver's license.
- Competitive hourly rate of $60.00.
