About the Role
The Residential Coordinator will provide case management services to adults with mental illness and substance abuse issues residing in our supportive training apartment program. This includes developing and implementing individualized service plans, facilitating support team meetings, and ensuring consumers receive proper medical care and linkages to vocational and educational opportunities.
Responsibilities
- Develop and implement individual service plans, service plan reviews, and monthly progress notes.
- Facilitate support team meetings and complete admission and discharge paperwork.
- Ensure consumers receive proper medical care and linkages to vocational and educational opportunities.
Requirements
- A High School Diploma and a passion for working with adults with mental health and substance abuse issues.
- Excellent writing, computer, organizational, and interpersonal skills.
- A clean, valid NYS driver's license with 3 years driving experience.
