We are seeking a qualified Licensed Healthcare Provider to join our team at Charter Oak Communities. As a key member of our healthcare team, you will provide direct medical care to residents at Scofield Manor. Your responsibilities will include conducting assessments, planning and administering medication, and supervising care.
Key Responsibilities:
- Assess resident health needs and develop individualized care plans.
- Order and administer medications, ensuring accuracy of records.
- Provide training and development opportunities for staff members.
- Maintain detailed records on each resident, adhering to state regulations.
- Communicate effectively with residents, families, staff, physicians, and other healthcare providers.
Qualifications:
- State of Connecticut Licensed Practical Nurse (LPN) certification required.
- Minimum four years LPN experience, preferably two in a managerial role.
- Strong knowledge of gerontology, medical, and mental health nursing.
- CPR certification mandatory.
