Job Summary
We are seeking an experienced licensed practical/vocational nurse (LPN/LVN) to join our team in providing community-based healthcare services to patients in Stamford and surrounding areas. In this role, you will work collaboratively with a multidisciplinary team to deliver high-quality care that meets the unique needs of each patient.

Duties and Responsibilities
- Provides high-quality nursing care to patients in their homes, following established protocols and procedures.
- Conducts thorough assessments of patients' physical and emotional needs, documenting findings in the electronic medical record.
- Develops and implements individualized plans of care that address patients' unique needs and goals.

Qualifications
- Current LPN licensure in the state of practice
- Current CPR certification
- At least one year of experience in home care or a related field.
