Job Description

We are seeking a highly skilled Family Nurse Practitioner to join our team in providing primary care services to the underserved community in Stamford and Greenwich.

Salary: $125,000 - $135,000 per year

Job Overview:

As a Family Nurse Practitioner, you will be responsible for delivering high-quality patient care to individuals of all ages, from infants to seniors. You will work closely with our interdisciplinary team to provide comprehensive primary care services, including routine check-ups, health screenings, and chronic disease management.

Responsibilities:

  • Provide direct patient care, including medical history, physical exams, and diagnosis
  • Develop and implement individualized treatment plans to meet the unique needs of each patient
  • Collaborate with healthcare professionals to ensure seamless care coordination
  • Stay up-to-date with the latest medical research, technologies, and best practices to deliver evidence-based care
  • Communicate effectively with patients, families, and healthcare teams to ensure clear understanding and positive outcomes

Requirements:

  • Master's degree in Nursing (MSN) or Doctoral degree in Nursing (DNP)
  • Certification as a Family Nurse Practitioner (FNP)
  • Current licensure as an Advanced Practice Registered Nurse (APRN)
  • Experience working with underserved populations preferred
  • Spanish language proficiency a plus

Work Schedule:

8 hours administrative time, 32 hours clinical time
