Oncology Nurse Navigator Job Summary

This is a full-time position in the Cancer Center Department, requiring 40 hours of work per week with 8-hour shifts between 7:30am-5:30pm. The role involves 50% coverage in Norwalk, CT, and 50% coverage in Danbury, CT.

Key Responsibilities:
1. Conduct case findings and screen cancer patients to identify support, education, discharge planning, and referral needs.
2. Conduct comprehensive assessments of patients' physical, functional, psychosocial, and financial status in cooperation with the healthcare team.
3. Develop, implement, and evaluate plans of care and support through an interdisciplinary process in conjunction with patients and families, linking them to appropriate institutional or community resources.
4. Procure services, serve as an advocate for patients and families with scarce resources, and facilitate referrals to state and local financial assistance agencies or programs.
5. Function as a liaison between the hospital, physicians, patients, and community resources to achieve maximum communication between all parties, ensuring coordinated care and meeting all patient needs.
6. Identify quality issues related to cancer diagnosis, treatment, and follow-up, risk management issues, and reimbursement issues, and refer them to appropriate committees or personnel for action or resolution.
7. Assess and evaluate cancer care through interaction with patients and surveys, and implement quality improvement projects as needed.

Requirements:
Bachelor's Degree required. RN CT Licensure Required. Oncology Certification and prior experience a plus.

Working Conditions:
Some manual skills/motor coordination and finger dexterity required. Some occupational risk involved. Medium to Heavy effort may be exerted, up to 35 lbs. force. Some exposure to dirt, odors, noise, human waste, etc.
